Schedule an HRT Appointment

To schedule your Hormone Replacement Therapy Consultation, please select the location you plan to visit from the options below. When you select your location, you will be redirected to complete your appointment booking.

Before Scheduling Your Appointment
1. Patients scheduling at Avon or Sandusky must complete their Hormone Replacement Therapy Evaluation Packet in full. Be sure to open each form and click “Submit,” even if no fields are required.
2. Complete your lab work or saliva testing.
3. Ensure your lab results have been submitted to the pharmacy so our pharmacists can review them and prepare for your consultation.

Important
For the Avon and Sandusky locations, consultations can only be scheduled after your packet is completed and lab results have been received. Appointments may be canceled if these steps are not completed in advance.

For the Perrysburg location, you may book your consultation prior to completing forms and labs.
